Why cannot records be used to obtain the recipient's ABO and Rh group for RBC transfusion?

Records cannot be used as the source of the patient's ABO and Rh group for RBC transfusion because if the records are wrong - and they may be - the patient could receive ABO-incompatible red cells and experience a potentially fatal hemolytic transfusion reaction.

This adverse consequences are illustrated by this case.
