What is the probable reason for this discrepancy?

In general, causes of a weak reverse grouping include:

  • hypogammaglobulinemia or agammaglobulinemia (hereditary or acquired)
  • low gamma globulin levels in a newborn or elderly patient

In this case the patient is 89 years old. This may explain the discrepancy.
